ABSTRACT

Objective:To analyze the correlation between the body composition and cardiorespiratory fitness (CRF) decline in physical examination population of different genders.Methods:Clinical data of the cardiopulmonary exercise testing (CPET) and body composition analysis of 439 people who received physical examination in the Medical Examination Center of Peking University Third Hospital from May 2021 to September 2021 were retrospectively analyzed. The general data, physical examination, biochemical parameters, body composition and CPET results were collected. The subjects were divided into normal group and decline group according to the percentage of peak oxygen uptake (VO 2peak) levels ≥ 85% or<85%. Multivariate logistic regression was applied to investigate the influencing factors of CRF decline in subjects of different genders. Results:Among men, total cholesterol and triglyceride in the decline group were significantly higher than those in the normal group [(5.097±0.890) vs (4.865±0.856) mmol/L, (1.778±1.200) vs (1.485±0.709) mmol/L], and the blood homocysteine (Hcy) and skeletal muscle index were significantly lower than those in the normal group [13.00 (11.30, 15.90) vs 13.80 (12.05, 17.10) μmol/L, (7.89±0.65) vs (8.08±0.64) kg/m 2] (all P<0.05). Among women, skeletal muscle index in the decline group was significantly lower than that in the normal group [(6.21±0.52) vs (6.53±0.56)kg/m 2], and percent body fat was significantly higher than that in the normal group [(32.83±4.92)% vs (31.21±4.55)%] (all P<0.05). The elevation of triglyceride level ( OR=1.487, 95% CI: 1.042-2.121) and visceral fat area ( OR=1.032, 95% CI: 1.014-1.051) were positively correlated with the decline of CRF in man, the decrease of skeletal muscle index ( OR=0.215, 95% CI: 0.106-0.435) and the increase of percent body fat ( OR=1.149, 95% CI: 1.060-1.245) were positively correlated with the decrease of CRF in women (all P<0.05). Conclusions:There is a correlation between body composition and CRF decline in physical examination population of different genders. Men should control visceral fat more effectively, and women should pay attention to increase muscle mass while reducing body fat, in order to improve CRF.

ABSTRACT

Objective To evaluate left ventricular geometry and function in elderly type 2 diabetic patients with normal and high pulse pressure by using three-dimensional speckle-tracking echocardiography (3DSTE).Methods A total of 76 elderly non-hypertensive type 2 diabetic patients were divided into type 2 diabetic combined with high pulse pressure group (n=36;24 h pulse pressure ≥60 mmHg) and type 2 diabetic combined with normal pulse pressure group (n=40).Forty age-and sex-matched healthy volunteers with normal 24 h pulse pressure were recruited as control group.3DSTE was performed in all participants.Results Significant differences in left ventricle geometry were found among the three groups (x2 =8.428,P=0.015),and concentric geometry was most prevalent in type 2 diabetic patients with high pulse pressure group.Global longitudinal strain in type 2 diabetic patients with normal pulse pressure group was significantly lower than that of control group (P<0.001).The type 2 diabetic combined with high pulse pressure group showed severely lower strain values in all directions compared with control group and type 2 diabetic combined with normal pulse pressure group (all P<0.05).Conclusion 3DSTEC can detect subclinical cardiac abnormalities in elderly type 2 diabetic patients with normal and high pulse pressure.

ABSTRACT

Objective To explore the effect of family health management on community type 2 diabetes medication compliance.Methods Totally 123 patients with type 2 diabetes who live in Shanghai City Qingpu District Xujing Town An Ju Wei Hao Xiao Qu and met the WHO diagnostic criteria for type 2 diabetes were enrolled into this study.The patients' age was 50-75 years,including 60 male,63 female cases,mean age was (63.9 ± 6.9),(10.0 ± 7.4) years,course of disease,a family health file,making personalized home health management measures of community were analyzed.The effects of the 2 years of intervention on the compliance of the patients before and after the intervention as well as the blood pressure,blood glucose,blood lipid control were evaluated.Results The treatment compliance rate was (77.1%,94/122,intervention 1 cases died) significantly higher after intervention than that before intervention (62.6%,77/123) (x2=6.06,P<0.05).After the intervention,the rate of target achievement in blood pressure (51.6%,63/122),blood glueose (44.3%,54/122),total cholesterol (72.1%,88/122),triglyceride (61.5%,75/122) low density lipoprotein (75.4%,92/122) were higher than those before intervention [blood pressure standard rate of 36.6% (45/123),blood glucose standard rate of 28.5% (35/123),total cholesterol standard rate of 59.3% (73/123),triglyceride standard rate of 47.9% (59/123),low density lipoprotein standard rate of 65.0% (80/123)],the differences were statistically significant (x2=5.63,6.61,4.44,4.51,3.97,P<0.05).Conclusion Community and family health management can improve the medication compliance in patients with type 2 diabetes.

ABSTRACT

At present, the numerical model of bone remodeling is inadequate to copy the physiological process of bone remodeling. According to the physiological mechanism of bone remodeling and the anisotropy of bone mechanics properties, an anisotropic bone modeling model with mechanostat was developed, and then was applied to a study on the remodeling evolution of 2D squareplate model. Good results were obtained. The model can be used in the study of implant, osteoporosis and other bone diseases.
